Hello, I'm Dad Pharmacist.
The weather has been getting chilly, and children are starting to catch colds while the flu is also going around these days.
When your child has a fever, a runny nose, and a cough all at once, most parents tend to think something like this.
"Since there are multiple symptoms, one dose of a multi-symptom cold medicine should do the trick, right?"
In fact, if you look at children's multi-symptom cold medicines, they contain not only ingredients for runny nose, cough, and phlegm, but also acetaminophen — the fever-reducing ingredient found in the Tylenol family of products.
However, there are cases where, after giving a multi-symptom cold medicine, the fever doesn't come down as well as you might expect.
You might then wonder whether your child's cold is particularly severe, or whether the medicine just isn't working well enough.
Let's go through this situation step by step and check what's going on.
What matters, though, is not simply "whether it's included" but "how much of it is included."
So let's check how much fever-reducing ingredient is actually in the 챔프 and 콜대원 multi-symptom cold medicines.
The table above compares products based on the dosage given to children of the same age.
You can see that the amount of acetaminophen differs between 챔프시럽, a single-ingredient fever reducer, and 챔프콜드펜, a multi-symptom cold medicine.
Let's take children aged 4 to 6 years old as an example.
챔프시럽, a single-ingredient acetaminophen fever reducer, is given 7.5mL per 1 dose, providing 240mg of acetaminophen.
However, for the same age group, the multi-symptom cold medicine 챔프콜드펜 is given 5mL per 1 dose, delivering 70mg of acetaminophen.
You can clearly see that the amount taken is quite different.
The situation is similar with 콜대원.
So what does the picture look like for 콜대원?
콜대원키즈콜드에스시럽 contains 150mg of acetaminophen per 1 sachet (10mL).
However, children aged 4 years and older but under 6 years take 2/3 of a sachet per 1 dose, so the actual amount of acetaminophen taken is approximately 100mg.
By contrast, the amount of acetaminophen in 콜대원 키즈펜 시럽 for this same age group is 240mg.
Just because a product name includes words like 'fever reduction' or 'fever relief' does not mean it contains the same amount of acetaminophen as a single-ingredient fever reducer.
In the end, we can confirm once again that even for the same age group of 4 to 6 years old, the actual amount of acetaminophen taken differs considerably between a single-ingredient fever reducer and a multi-symptom cold medicine.
It's not that multi-symptom cold medicines are weak
Seeing this, you might wonder, "So is a combination cold medicine a weak medicine?"
That's not what this means.
Multi-symptom cold medicines were never designed solely to bring down a fever.
They also address runny nose,
and help reduce coughing and phlegm,
to also relieve symptoms like fever and body aches,
it is a medicine that combines multiple ingredients all at once.
As a result, when you look at just the fever-reducing ingredient, you can think of it as being included in a smaller amount than the age-based recommended dose of a single fever reducer.
So if your child's main symptom is a high fever rather than a runny nose or cough, you may find yourself thinking, "I gave a combination cold medicine, so why isn't the fever coming down?"
So should you simply add a fever reducer right away?
From this point on, you need to be a little careful.
Because acetaminophen is already included in combination cold medicines,
if you want to add a fever reducer, you need to factor in the amount already taken earlier.
In particular, for acetaminophen, rather than looking only at your child's age,
if you know their weight, it is more appropriate to calculate based on 1 dose at 10–15mg per kg.
For example, even among children who are all 6 years old, it would be hard to say that a child weighing 17kg and a child weighing 22kg need exactly the same amount of fever reducer.
So if your child still has a fever after taking a combination cold medicine, the answer isn't simply "one more sachet of 챔프." Instead,
how many mg of acetaminophen were in the cold medicine just taken,
how many kg the child weighs,
and how much time has passed since the last dose—
it's a good idea to check all of these together.
The additional amount that could be taken based on age alone can be calculated as follows.
However, as mentioned earlier, calculating by weight is more accurate!
Please keep this in mind once more!
Combination cold medicines do contain a fever reducer.
However, compared to taking a single fever reducer, the amount of acetaminophen is often smaller when judged by the same age standard.
So if fever is the main symptom, the fever-reducing effect may feel weaker than expected when only a combination cold medicine is given.
On the other hand, what if you give more combination cold medicine because the fever won't come down?
Because the runny-nose and cough ingredients also increase at the same time, arbitrarily increasing the dose is not a good idea either.
When your child has a fever, a runny nose, and a cough all at once, rather than asking "Does the combination cold medicine contain a fever reducer?", it is better to ask "How many mg of acetaminophen did my child actually take?"
Checking up to this point will make things much easier to understand.
Does this apply to all combination cold medicines?
Besides 챔프 and 콜대원, there are also children's combination cold medicines such as 판콜아이콜드 and 화콜키즈콜드.
If you calculate the content amounts, you can consider the situation to be the same.
Likewise, please keep in mind that if your child has a high fever, the fever-reducing ingredient may be insufficient.
